Differences between protocol and review

The protocol was originally written in RevMan 4.2. This systematic review was written in RevMan 4.2 and later transferred to RevMan 5.0 during the peer review procedure. We have included risk of bias table for each included study and a summary of findings table that were not described in the original protocol.
