Felix Ram FR (previous author), PB and SR wrote the draft protocol and the final review. FR and SR selected trials for inclusion, trials were reviewed and quality assessment carried out by FR & SR. Outcome data was extracted by FR & SR. The draft manuscript was written by FR, SR and and the final manuscript was written by FR, SR & PB for the Cochrane Library. FR & SR updated the review in December 1998, FR sought further studies in December 1999, none found. FR reformatted the review from version Revman 3.1 to Revman 4.0.4. October 2003, Review updated by FR and JP in May 2005 (FR no longer involved in the review).
