The lead review author (RWA) initiated and planned the review, including writing the protocol.

The lead review author (RWA) planned the search strategy and two review authors (RWA and KAH) will undertake the search. Updated searches will be done by the Cochrane Back Review Group's Trials Search Co‐Ordinator, Rachel Couban.

Three review authors will participate in the decision‐making process regarding inclusion and exclusion of trials (RWA, KAH, BHS).

All review authors will participate in the data extraction and assessment of risk of bias of included studies.

The lead review author (RWA) will be sending progressive drafts of the review to all co‐authors and to our mentor, Andrew Oxman, for comment and input.
