I

Tumour confined to the bile duct, with extension up to the muscle layer or fibrous tissue.

II

Tumour invaded beyond the wall of the bile duct to surrounding adipose tissue.

Tumour invaded adjacent hepatic parenchyma.

IIIA

Tumour invaded unilateral branches of the portal vein or hepatic artery.

IIIB

Regional lymph node metastasis (including nodes along the cystic duct, common bile duct, hepatic artery, and portal vein).

IVA

Tumour invades main portal vein or its branches bilaterally; or the common hepatic artery; or the second‐order biliary radicals with contralateral portal vein or hepatic artery involvement.

IVB

Metastasis to periaortic, pericaval, superior mesenteric artery, celiac artery lymph nodes, or a combination of these.

Distant metastasis present.

Distal bile duct tumours

Tumour confined to the bile duct histologically.

IB

Tumour invaded beyond the wall of the bile duct.

IIA

Tumour invaded the gallbladder, pancreas, duodenum, or other adjacent organs without involvement of the celiac axis, or the superior mesenteric artery.

IIB

Regional lymph node metastasis.

III

Tumour involves the celiac axis or superior mesenteric artery.

IV

Distant metastasis.
