Contributions of authors
In addition to the contributions stated above, NJ will write the protocol and review, construct the forest plot and review the trials. TM will have the primary responsibility for data extraction, sourcing expert reviews and data integrity. Both NJ and TM will examine the library searches independently and select relevant trials. PC and EG with review the analysis, select stratification variables, check data integrity and contribute substantially to the writing.
