Differences between protocol and review

The outcomes have been divided into primary and secondary outcomes. Quality of life has been added as an outcome. The assessment of methodological quality of studies, hence risk of bias, has been revised in line with the recommendations of the Cochrane Handbook (Higgins 2009).
