Differences between protocol and review

According to the new version of the Cochrane Handbook for Systematic Reviews of Interventions (Higgins 2011), we have replaced the 'Quality assessment of included studies' in the original version with 'Assessment of risk of bias in included studies'.

Notes

This protocol was withdrawn from The Cochrane Library in Issue 9, 2010, as the review authors were not able to write the review. A new team have subsequently updated this review.
