Differences between protocol and review

ZETOC and Dissertation abstracts were not searched as these database are no longer accessible to the Cochrane Anaesthesia Review Group Trials Search Co‐ordinator who helped run the searches.
We included only randomized, double‐blind placebo‐controlled trials in the review. This change was suggested at peer review stage. We therefore did not search for, or include, case controlled and quasi‐randomized trials in our review.
We used the 'Risk of bias' tables for assessment of bias.
We clarified the types of participants to include all patients with adrenal insufficiency, whether moderate or chronic.
The updated review was conducted in accordance with the latest version (5.1.0) of the Cochrane Handbook for Systematic Reviews of Interventions (Higgins 2011). A new author (Anna Wrzosek) was involved in updating the review.
