Dr Halfhide co‐ordinated the changing of the protocol into a full review. This included undertaking searches, appraising quality of papers, data extraction, entering data into RevMan, analysis, interpretation of data and writing the review. Dr Halfhide acts as guarantor of the review.

Dr Couriel originally conceived the idea for the protocol and review. He was the lead author of the protocol. He provided advice throughout the selection of the eligible studies, data interpretation and the writing of the review.

Dr Evans provided useful comments and ideas throughout conception of the protocol. During the review she provided help with appraising the quality of the studies, data extraction and interpretation of results.

Differences between protocol and review

There was a post hoc change to the types of bronchodilators to be included in the review. There is now a fourth category 'long‐acting anticholinergics agents (tiotropium)' due to the fact that these preparations are now commercially available.
