Contributions of authors

Kameshwar Prasad was responsible for every aspect of the first version of the published review, from conception, through protocol development, trial identification, data abstraction, and writing it. In the update, he has conceived and co‐ordinated all the stages of the review. He made visits to Newcastle, UK, to forge links, hold discussions and collect data from the co‐authors of this update. He led the assessment of trial quality, entry of the data into RevMan, the analysis and interpretation of the data. He wrote the first draft of the update and finalised the review.

Barbara Gregson cross‐checked all the data, participated in the quality assessment of the trials, re‐analysed the STICH data for the purpose of the update, read the first draft and provided input for the final draft of the review.

Professor Mendelow provided the STICH trial data, participated in the interpretation of the data, checked all the data, contacted the authors of some of the trials for more data, read the drafts and provided input for the final draft of the review.
