VSK and KM wrote the protocol and are responsible for searching for trials and data extraction, quality assessment and development of the review. VSK will contact the original authors for missing data and data input.

KM and SM conceived the idea for this systematic review and have co‐ordinated its development. KM will screen search results, select articles for inclusion, assess trial eligibility and assess trial quality.

SM will resolve disagreements in trial selection, assess trial quality and extract data.

DC will extract data, conduct data analysis and assist in writing the final review.

IAH will supervise all stages of the review and act as guarantor to ensure the review meets the standards for publication.
