SWN developed the search strategy and searched in multiple databases for eligible papers together with IDH. The full text of these papers were retrieved and further assessed for eligibility by SWN and IDH independently in consultation with a colleague (JDZ). SWN and IDH extracted data on to a previously designed data extraction sheet. Data entry was cross checked by SWN. Methodological quality was assessed by SWN and IDH. IDH reviewed the final draft.
