Reason for withdrawal from publication

Minor inconsistencies between the Description of studies section and Characteristics of included studies table have been corrected in Issue 1, 2006. This version of the review has been withdrawn in Issue 4 2009 until the update is published. The update is expected to be published in Issue 1, 2010 of the Cochrane Library.
